The grounding of pilot vessel Takitimu II near Bluff has prompted a Transport Accident Investigation Commission (TAIC) investigation, and its findings carry a clear message well beyond commercial shipping: navigation techniques must adapt when conditions change.
Takitimu II departed South Port on 26 December 2024 to rendezvous with an inbound ship offshore. On board were the vessel's master, a deckhand, and a harbour pilot scheduled for an offshore transfer. After clearing Stirling Point, the vessel altered course too far to starboard and struck rocks at an estimated speed of 18 to 20 knots. Two of the three people on board sustained minor injuries, and the hull and underwater fittings suffered moderate damage. No pollution occurred. Coastguard subsequently refloated Takitimu II and towed it back to port.
TAIC's final report ruled out mechanical failure, distraction, and medical issues as contributing factors. The investigation pointed instead to a drop in situational awareness driven by a combination of patchy fog, vessel speed, and a reliance on visual navigation at precisely the moment when visual cues could no longer be fully trusted.

"Speed and route choice reduced the time available to detect and correct course," said TAIC Chief Investigator of Accidents Louise Cook. "The vessel's electronic navigation systems were switched on and available to confirm the vessel's position and progress, but the master wasn't referring to them."
Although the fog stopped short of completely obscuring channel buoys and beacons, the report concluded it was enough to undermine their reliability as primary navigation references. Electronic systems were available but were not being actively cross-checked.

"The key point is that safe navigation means choosing techniques that fit the conditions," Cook said.
The investigation also examined South Port's training and familiarisation processes for pilot vessel masters. The masters involved were experienced operators, yet no formal mechanism existed to periodically verify ongoing navigation proficiency, particularly when conditions moved outside the routine.
"When people do a job every day in routine conditions and nothing bad happens, it's easy for risky habits to become normal," Cook said. "Unusual conditions are where those habits are tested."
After the grounding, South Port reviewed its operational systems and introduced a series of changes. These included regular navigation competency assessments, updated passage plans and operating procedures, and refresher training with a specific focus on the disciplined use of electronic navigation. TAIC considers those actions adequately address the safety issues identified in its inquiry.

Although Takitimu II was a commercial vessel, TAIC notes the broader relevance of the findings. Operating small, fast boats in confined coastal waters leaves little margin to recover when something goes wrong, and reduced visibility shrinks that margin further. Combining electronic navigation with visual techniques, and maintaining proficiency through regular practice, gives crews more time to recognise a developing risk and correct course before an accident occurs.
